The forms below need to be filled out in order to enroll at St. Joseph the Worker Grade School. If your child is transferring from another school, you will also need to fill out the record request form.
Other items needed to enroll at St. Joseph the Worker Grade School are: Birth Certificate from the state your child was born in (not the hospital record), updated shot records, social security number and a baptisimal certificate (if your child was baptized). There is also a non-refundable $45.00 registration fee.
***** State requirement for the 2018-19 school year: all students entering Pre-school 3 or 4 MUST be that age by July 1.
