Health Center and Immunization Information

 The information listed below is for the school nurse, the school health center and 7th grade immunization information.

The form below is the new requirements on immunizations starting with the 2013-14 school year. Please make sure you read this so your child is up-to-date when school starts in August. 

If you have any questions, please call your doctor's office. You may also call the school and we will check with the school nurse to answers any questions you may have.

SCHOOL HEALTH CENTER

 The School Based Health Program began with the 2005-06 school year. The program is generously funded by a grant from The Sisters of Saint Joseph Health and Wellness Foundation and provides a nurse, guidance counselor, and a clerical aid position for the school.

Services provided for students and their families include:

• Preventative Health Screenings
• Basic First Aid
• Classroom Education

Counseling

• Individual and Family Counseling
• Group Sessions
• Individual Mental Health Assessments
• Classroom Education

In addition, services also provided for the parish and community include:

• Fall Health Fair
• Health Screenings
  • Vision
  • Dental
  • Hearing
  • Blood Pressure Checks
  • Height/Weight/Body Mass Index Monitoring
  • Scoliosis
• Healthy Nutrition Education

Health screening provided to St. Joseph School also.
