The form below is the new requirements on immunizations starting with the 2013-14 school year. Please make sure you read this so your child is up-to-date when school starts in August.
If you have any questions, please call your doctor's office. You may also call the school and we will check with the school nurse to answers any questions you may have.
SCHOOL HEALTH CENTER
The School Based Health Program began with the 2005-06 school year. The program is generously funded by a grant from The Sisters of Saint Joseph Health and Wellness Foundation and provides a nurse, guidance counselor, and a clerical aid position for the school.

- Healthy Nutrition Education
Health screening provided to St. Joseph School also.
